Default FAA form 8130-6 help

I've completed my RV-12 ELSA and trying to pull together all the paper work and it's not clear to me how to complete his form. It looks like some things get completed, sent in to the local FSDO and the rest is completed at time of inspection. Can someone share a completed 8130-6?

Are you sure that 8130-6 is the correct form? Mel?

Yes, I already have my registration card, so next is the inspection. From the FSDO office the list of documents required are:
- 8130-6 Completed Application for U.S, Airworthiness Certificate
- 8050-3 US Aircraft Registration
- Program Letter (For the purpose of the certificate)
- 8130-15 Statement of Compliance
- 8050-2 Aircraft Bill of Sale
- Weight & Balance with fwd and aft CG established
- Three view drawing or photos of the aircraft
- Area of operations
- Manufactures assembly instructions
- Aircraft Operating Instructions
- Aircraft Maintenance and Inspection Procedures
- Aircraft Flight Training Supplement (Approved Flight Test Procedures must be with the A/C, specific to that Aircraft)
- Validated the consensus standard listed on the 8130-15
- Log Books are signed off in a condition for safe flight
- Review the records for compliance of any applicable AD's
