Lycoming Engine Inspection and Overhaul Form

The Lycoming Overhaul Manual 3-19 talks about an "Engine Inspection and Overhaul Form" that is used as a log and also a checklist for the overhaul process.

The Google did not turn up a copy of this form. I imagine it is a lengthy form that leads the re builder through the step by step of disassembly, inspection, and reassembly.

Does anyone happen to have a copy of this form that they can share?

Does anyone have a checklist of sorts for an engine tear down/inspection/reassembly?

Of course the overhaul manual is a good step by step however it is not arranged like a checklist for fast and comprehensive review of the project...so I imagine someone has made up a checklist so that I do not need to reinvent the wheel on this...please...
